State Mandated Immunization Requirement Notice

 

Dear Eisenhower Middle School Parents and Guardians:

Since your child will be entering 7th grade in August, we want you to be aware of 2 immunizations that will be required prior to their first day of school. 

According to the New Jersey state law, students entering 7th grade are required to have the following immunizations: 

Diphtheria, Tetanus and Pertussis  (N.J.A.C. 8:57-4.10) -

One (1) booster dose of the tetanus, diphtheria, and pertussis vaccine (tdap) given no earlier than their 10th birthday 

Meningitis  (N.J.A.C. 8:57-4.20)  - 

One (1) dose of the meningococcal vaccine

It is required that documentation of these two immunizations or the date/time of an appointment for your child to receive these immunizations (on doctor’s script or letterhead) must be provided to the school nurse by August.
